YouthCare Transitions
Transitions Programs are designed to support teenagers as they develop into responsible and independent young adults at home, at work, and in the community. The programs focus on development of self-awareness, social thinking, executive function, and life skills including career explorations and post-secondary exploration. Based on the premise that youth learn best through "real-life" experiences, the key component of each Transitions Program is community-based education. Through these experiences, teens have the opportunity to practice daily living skills in natural environments with the support of our talented and highly trained staff.

NEW!! TUESDAY NIGHT TV DINNER GROUP (February 2-May 25, 2010)
Transitions: TV Dinner Group is a weekly therapeutic group offering teens aged 15-19 the opportunity to learn valuable communication and perspective-taking skills through viewing and discussion of current television shows as well as participation in other engaging group activities. SATURDAY EXPLORATIONS FOR TEENS (Sessions: Winter/Spring 2010 and Fall 2010)
Transitions: Saturday Explorations is a therapeutic skill-focused Saturday program for teens aged 13-19. If offers teens fun recreational experiences while learning a variety of practical life skills needed for independence. Teens take part in daily living skill instruction (e.g. communication, money management, public transportation, safety, leisure planning), a field trip each Saturday, team building activities, therapeutic group activities and much more. CAREER SKILL DEVELOPMENT FOR TEENS
Transitions: Career Skill Development is a six-week summer program that exposes teens aged 15-19 to community-based career and post-secondary exploration opportunities. It is targeted at teens ready for exposure to the real work world through an internship experience with the help of supportive job coaches. Teens additionally take part in career planning and skill instruction (e.g. time management, interview skills, personal presentation), team building activities, educational speaker presentations, social thinking groups, and community lunches. SUMMER EXPLORATIONS FOR TEENS
Transitions: Summer Explorations is a six-week therapeutic skill-focused summer program for teens aged 13-16 which provides a fun-filled group experience in a structured and supportive setting. In this program, teens develop daily living skills through direct instruction, planning and completion of an overnight trip and weekly field trips, community service work, group problem-solving and ropes course activities, electronic diary use, as well as meal planning and preparation.
