IV tPA is a class 1 level A recommendation from the major healthcare scientific organizations who have reviewed the evidence supporting its use. Systems should be in place to insure adherence to recommended standards for Primary Stroke Centers or equivalent State Stroke Designation programs. Every patient or caregiver should have the potential risks and benefits of IV tPA explained prior to therapy whenever possible, and the following written consent form is one example of this. However, delivery of therapy should never be delayed in order to obtain written informed consent.
