Primary Care
MGH Senior Health provides office-based Primary Care and consultation for adults over the age of 65. We provide patient and family centered care in the context of each individual's functional and psychosocial situation. Our team, which includes specialists in geriatric medicine, geriatric psychiatry, rehabilitation medicine, geriatric nursing, and social work, develops a comprehensive care plan for each patient. Emphasis is placed on wellness promotion and improvement in daily functioning. Our staff coordinates your care across the continuum, which may include hospitalization, rehabilitation, nursing home, and home care services.

New Patient Folder

Maureen and Pattie at the front desk will mail you a New Patient Folder including information about your visit, parking, communicating with the office, and your Doctor's profile.

 

 

 

Education is a cornerstone of our care. Patients and families may browse in our comfortable, on-site Patient Resource Center that has a variety of informational pamphlets, books, and videos on prevention, wellness, and aspects of aging.

Consultation Services
We offer consultation on specific medical problems, such as:

For consultations only, we provide both you and your referring doctor a detailed report of our findings and recommendations.

Inpatient Services

We admit our patients to Massachusetts General Hospital where we are on staff as attending physicians. There are circumstances, like an emergency, when the ambulance will take you to the closest hospital for evaluation and stabilization. If appropriate and possible we would be happy to assist in your transfer to the MGH.

If you are admitted to the MGH your Senior Health physician or member of our team will follow you during your hospital stay. If you are admitted by a specialist, such as a Cardiologist, please notify us of your admission so that we can continue to follow you during your hospital stay. MGH is a teaching hospital, and we are pleased to work in collaboration with physicians in training who will see you in addition to our staff.

Upon discharge from the hospital we request that you make a follow up appointment with your physician or our nurse practitioners. Please call us at 617-726-4600.

Skilled Nursing Care and Rehabilitation Services

If you require post hospital care in a rehabilitation or skilled nursing facility a member of our practice will continue to treat you if you are discharged to one of the following facilities: Spaulding Rehabilitation Center-Boston, Eastpointe Rehabilitation Center-Chelsea, The North End sub –acute or Long term care units at the North End Nursing Home-Boston, Clark House-Westwood and LaSell Village in Newton.

Upon discharge from any care facility we request that you make a follow up appointment with your physician or our nurse practitioners. Please call us at 617-726-4600.

Geriatric Psychiatry

Our practice offers the expertise of a geriatric psychiatrist who works with our primary care physicians to diagnose and treat conditions such as depression, memory problems, dementia, anxiety, stress, coping with chronic disease and terminal illness. Services include psychopharmacology, psychotherapy and consultation to the team.


Social Services

Geriatric social workers are integral members of the team who work with patients, family members, and caregivers. Services include individual counseling for patients, caregiver support, and care coordination. Areas of discussion may include home health care, nursing home and assisted living options, caregiver stress, and coping with the emotional challenges of aging.
