Preparing for your visit to the Allergy Associates
Please arrive at least 15 minutes prior to your scheduled appointment time
At each office visit you will be asked to provide your MGH-hospital blue card, your insurance card, your referral – if your insurance is an HMO/PPO – and your co-payment, if applicable. This is required at the time of the visit (cash, check and Visa or MasterCard accepted).
Registering as a Mass General Patient
All Mass General patients must have a current blue MGH registration card for hospital services. You may get one at Patient Registration on the first floor of the Wang Building, next to the outpatient pharmacy, Monday through Friday from 8 am to 4:30 pm. Please arrive at least 30 minutes prior to your scheduled appointment if you need to register for a blue card. Remember to bring all of your insurance cards with you. You may also register in advance by phone by calling Patient Registration at 866-211-6588 (hours are Monday through Friday, 8 am to 8 pm). For all visits, please remember to bring your blue hospital card.
Getting a Referral
A majority of patients seen in our practice need a referral from their primary care provider. Please consult with your insurance carrier prior to your appointment. If a referral is needed but not obtained, you will be responsible for payment.
Your Allergy Associates Healthcare Team
You will be evaluated and cared for by physicians who have received specialized training in the field of Allergy and Clinical Immunology. As MGH is a teaching hospital affiliated with Harvard Medical School, doctors receiving advanced subspecialty medical training, such as residents and fellows, may also work with your physician.
Registered Nurses
The nursing staff is adept at caring for patients with allergic and immunologic diseases, including the performance of allergy skin testing, pulmonary function testing and allergen injection immunotherapy. Your nurse works closely with other members of the team to help you to understand your diagnosis and treatment, manage medications and possible side effects, effectively care for yourself, and prevent medical problems. Also, they can act as case managers, helping you navigate the various services here at Mass General Hospital as well as beyond.
What to Bring to Allergy Associates
- New patient forms(if you are a new patient). Available here or at the time of your visit.
- Insurance card
- Prescription plan card
- Co-payment
- Referral. It is the patient’s responsibility to obtain a referral from a Primary Care Physician as patients who are seen without a referral will be asked to sign a waiver and may be billed directly.
- Blue Card. To register as a Mass General patient and get your MGH blue card that lists important contact and insurance information, please call: 866-211-6588.
- If you will be undergoing skin testing, please see Preparing for Skin Testing.
You may also receive your blue card in advance and in the mail be calling Patient Registration at 866-211-6588 (M-F, 8am to 8pm). Allow 7-10 days for delivery.

At each office visit you will be asked to provide your MGH blue card, your insurance card, your referral – if your insurance is an HMO. You will also be asked to provide your co-payment, if applicable. This is required at the time of the visit, your payments can be made in cash, check, Visa, MasterCard, or Debit.
All Mass General patients must have a current blue MGH registration card for hospital services. You may get one at Patient Registration located on the second floor of the Yawkey building adjacent to the rheumatology suite, Monday through Friday 8am to 4:30pm. Please arrive to your appointment at least 15 minutes early if you need to register for a blue card.
Your Rheumatology Healthcare Team
You will be evaluated and cared for by physicians who have received specialized training in the field of Rheumatology. As Mass General Hospital is a teaching hospital affiliated with Harvard Medical School, doctors receiving advanced subspecialty medical training, such as residents and fellows, may also work with your physician. A registered nurse is available for procedures and other assistance.
