What
we do (ElderCare Services)
PrimeCare is a new medically focused elder care program developed to meet
the growing care needs of elders and their caregivers. We understand the
issues faced by elders and their caregivers are often complex, emotional
in nature and even overwhelming at times. There may be several different
types of senior issues to resolve and manage at once, in combination directly
affecting the health, well-being and safety of the elder. In addition,
important and often complex insurance, financial and legal matters may
further complicate already stressful elder care situations.
Our nurse care managers work with you and your elder to solve these
and other elder care issues that you may face. We can provide concrete
resources, elder care plans, options and caregiver support to ensure the
well-being of the elder, while making it easier for caregivers to maintain
a healthy balance in their lives. One of the keys to successful care of
an elder is ensuring that the caregiver can sustain the time and energy
needed for their family, career and other life interests.
How we do it
We start by gaining a full understanding of the problems
and concerns faced by the elder and caregiver. Our nurse care manager
will arrange a free consultation meeting with the caregiver and/or elder,
and this may be in your home, the elder’s home, or our office –
whatever is most convenient for you. The care manager will gather information
about the elder, accurately assess the elder care needs and then recommend
appropriate care management services. These services may be Targeted
at specific needs, i.e. assisted living decisions, or more broad in nature,
such as our Comprehensive Geriatric Assessment service
that begins with a 1-2 hour in-home evaluation of the elders:

social and psychological well-being |

safety for seniors in the home and in daily living |

elder care needs around financial and legal planning, insurance
coverage |

caregiver’s support and needs |
Following the geriatric evaluation
you will receive a comprehensive and detailed written report that includes
an individualized and prioritized elder care plan, detailing the necessary
elder care steps and resources available to assist you.
The PrimeCare senior plan is customized for the elder and caregiver. The
plan identifies the elder’s care needs and provides step-by-step,
issue-by-issue direction and guidance to help you keep your elder safe,
secure and as independent as possible. Included in the geriatric assessment
report will be our specific elder care guidance and recommendations for
managing care costs, spending money wisely, etc. Some of our clients use
the plan as a guide to implement the care steps themselves. Others prefer
to have PrimeCare help them carry out the more complex or “key to
success” components of the plan. Still others have PrimeCare implement
all the components of the elder care plan. The choice is yours. Regardless
of the option you select, we will make sure you have the information,
tools and caregiver support you need to have the plan work for the elder
and you.
Our Staff
PrimeCare care managers are experienced healthcare professionals with
expertise in both nursing and geriatric care management. Our registered
nurses have years of experience in patient care and special insight into
patient needs. As advocates for patients, they are able to navigate the
complex worlds of healthcare and insurance, effectively communicating
with physicians, pharmacists and other providers to secure the care that
your elder needs. In addition, they have years of experience in elder
care management, assisting elders and families with the unique challenges
they face and in finding the senior health resources and help they need.
Finally, our care managers have access to the expert resources of the
Massachusetts General Hospital and are members of the National Association
of Geriatric Care Managers.
