Services for elders and those who care for them

"My father fell last week at home. He was alone and laid on the floor for hours. He’s having trouble with his memory and sometimes his diabetes is out of control. He has medicines to help, but he doesn’t seem to take them when he should. When I talk with him about it, even I can’t seem to figure out what he should be taking, or when. His medicines are expensive too, and I’m not sure whether his insurance should pay for them. To make matters worse, he’s losing weight because he isn’t eating as he should. He’s not safe, and if I don’t do something soon he’ll get hurt and be unable to stay in the home he loves. I don’t know where to begin, or even how I will do what must be done - I have a job and a family of my own to care for."

PrimeCare is here to help by offering elder care management services and assistance to elders and caregivers facing such challenges. We will help you with:

Coordination of medical care for Elders
Home safety for seniors and support
Alternate living decisions
Guidance on financial, legal and insurance matters
Caregiver support and guidance

As our aged population continues to grow in number, with people living often healthier and longer lives, scenarios such as this will unfortunately become more and more common. Elders wishing to live at home where they feel secure and comfortable, yet at risk from medical, physical and mental decline, and even the home they love so much. Family and friends trying to help may suddenly be faced with new “caregiver” responsibilities and choices that can be complex, time consuming and emotional in nature. For caregivers these new responsibilities may further stress already very hectic and demanding lives. Sometimes, there just doesn’t seem to be enough information, time or energy to do what you know should be done for your loved one.
