to choose a primary care physician, please make sure to fill in the name of
your MGH Chelsea doctor.

We request that you please make any required payments or co-payments at the
time of your visit. We accept cash, personal checks, and credit cards. If
you have a question about your bill, please call (617) 889-8507 between 8:30
a.m. and 5:00 p.m.
