• Letter of Intent
• 3 letters of recommendation along with applicant recommendation request form (doc) .

  (One should be from someone who can comment on the candidate's ability to interact with patients/healthcare team)

• College Transcripts
• Massachusetts General Hospital Application Supplement , sent to Residency Program Director
• On-Site Interview by invitation
• Be registered in the National Matching Service (www.natmatch.com/ashprmp/)
• Be eligible for pharmacist licensure in Massachusetts Professional Credential Services MA Board of Registration in Pharmacy

The application deadline for RECEIPT of materials for the 2012-2013 residency is January 4, 2012. The PGY1 General and PGY2 residencies will begin July 2, 2012.
The PGY1 International program will start January 1, 2013.
