Indications:

Hypertension
Dissecting Aortic Aneurysm

Administration Guidelines:

Usual Dosage Range and Route:

LOADING DOSE : 2.5 - 10 mg over 2 minutes repeated at 5 minute intervals or for aggressive control may double dose at 5 minute intervals until desired response is achieved

*initial or loading dose is usually given prior to starting maintenance*

MAINTENANCE : 10 to 20 mg / hour initially, then slowly titrated up to 40 to 180 mg / hour as per patient response or 2.5 to 10 mg ivp every 15 minutes to 1 hour as needed.

Standard Concentrations:

500 mg / 250 ml

1250 mg / 250 ml

Syringe based, variable flow, continuous microinfusioin pump: 250 mg / 50 ml

EW Standard

500 mg / 250 ml

Special Considerations:

- PA - line monitoring of cardiac output, cardiac index, pcwp, CVP, SVR, is encouraged during maintenance infusions

- intubated patients should have peak inspiratory pressures evaluated periodically

Precautions and Side Effects:

- bradycardia, AV block

- bronchospasm

- hypotension

- cardiac failure

- CNS depression, hallucinations
