Indications:

Hyperglycemia
Diabetic ketoacidosis
Hyperkalemia

Administration Guidelines:
Human insulin only
Route (IV or SC) should be specified.

Usual Dosage Range and Route:

LOADING DOSE : 5 to 20 Units IVP

*loading dose is usually given prior to starting maintenance*

MAINTENANCE : 2 to 25 Units / hr IV titrated according to blood glucose level once under control, patients may be changed to a sliding scale coverage when appropriate.

Standard Concentration:

100 Units / 100 ml NS

Syringe based, variable flow, continuous microinfusion pump: 50 Units / 50 ml

EW Standard

100 Units / 100 ml NS

Special Considerations:

- Regular insulin (CZI) is the only insulin that can be administered intravenously

- Do not filter, insert infusion into y-site below any in-line filters

Precautions and Side Effects:

- Hypoglycemia

- Monitor serum electrolytes and blood glucose

- Blood glucose should be monitored every two hours until a stable dosage has been achieved. Consideration should be given to possible changes in serum electrolytes as serum glucose is stabilized.
