Universal Fellowship Application

Thank you for your interest in our Hand Surgery Fellowship Program. Please note that we have recently combined the Plastics and Orthopaedics Hand Fellowships into one program, and plan to accept one Plastics and two Orthopaedics trainees, and are currently reviewing applications for the 2012-2013 training period. Please complete the Universal Fellowship Application (you can find it at www.assh.org or use the link above). With it, please submit your curriculum vitae, a photograph, transcripts and three letters of recommendation (to be sent separately), one of which should be from the Chief of Service under whom you are currently training. All applications should be completed and received at the following address by December 31, in order to be considered for an interview:
