Pre-Surgery Preparations

The pre-surgery preparation process begins at your surgeon’s office. If you need labs and an ECG before surgery, your surgeon will arrange for these. If you are 50 or older, you need to have an ECG within one year of your surgical date.

To start your pre-surgical assesment process, please fill out your medical history using our online system - One Medical Passport. Click here to access One Medical Passport. We recommend that you enter your medical history through One Medical Passport as soon as your surgery is scheduled. Your surgery date will not be confirmed until the online questionnaire is complete. Once you complete the questionnaire, our Pre-Surgical Assessment Nurse will be able to access the information, and it will assist the nurse in organizing and documenting your complete medical history to prepare for your surgery. Someone at the ASC will contat you 3-10 days before your surgery date to review the information.
