When you arrive, please check in with the receptionist.
You will find it helpful to bring a date book
or calendar to keep track of your appointments
and tests. Also, we encourage you to bring someone
with you. At your first visit, you will also be given a new patient brochure.
Ideally, you will know in advance the duration
of your visit. We make every effort to keep to
the schedule, but unforeseen delays do occur.
In the event you do have to wait, please refer
to the “While You Are Here” section
of this booklet for ideas on how to help pass
You may be asked to complete paperwork while
you are waiting for your provider.
Otherwise, please have a seat in our waiting
room until your name is called. Please note that
we do have multiple clinical services on our unit
and some patients may be seen in a different order
than their arrival.
After your appointment, please check out with
the secretary and schedule your next appointment
or tests, if needed.
To change your appointment for any reason please
call the office at (617) 726-3906 as early as
possible, but at least 24 hours in advance.
Monday through Friday 8:30 am to 5 pm
At each office visit you will be asked to provide
your hospital blue card, your insurance card,
your referral – if your insurance is an
HMO/PPO – and your co-payment, if applicable.
This is required at the time of the visit. (Cash,
check and Visa or MasterCard accepted)
All MGH outpatients must have a current blue
hospital registration card for hospital services.
You may get one at Patient Registration on the
first floor of the Wang Building, next to the
outpatient pharmacy, Monday through Friday from
8 am to 4:30 pm. Please arrive at least 15 minutes
prior to your scheduled appointment if you need
to register for a blue card. Remember to bring
all of your insurance cards with you. You may
also register in advance by phone by calling
Patient Registration at 866-211-6588 (hours are
Monday through Friday, 8 am to 8 pm). For all
visits, please remember to bring your blue hospital
Some patients seen in our practice (except for
the GID/STD clinic) need a referral from their
primary care provider. Please consult with your
insurance carrier prior to your appointment. If
a referral is needed but not obtained, you will
be responsible for payment. If you have questions
