If you have recent dental x-rays or Panorex films , please bring them with you to your visit. In some cases, x-rays do not have to be repeated, limiting cost and exposure to the patient.
All patients seeing a Doctor for the first time will be required to complete a medical history form. This form will help identify important conditions that will be considered in planning your treatments. It is important that you complete this form completely and accurately in order to ensure the best quality care.
In addition to the medical history form, all patients are now required by law to sign a HIPAA form . HIPAA stands for the Health Information Portability and Accountability Act, a landmark piece of legislation enacted by Congress in 1996.
HIPAA protects a person’s medical record from unauthorized use and ensures confidentiality. By signing this form you are acknowledging that your medical information will be used in specific ways to treat your condition, to process any insurance payments, and may be disclosed to office staff as part of clinical operations. Other uses, such as for marketing health services or disclosure to friends and family, are restricted by law and require your approval to do so.
