Hernia Surgery

A hernia is a defect in the musculature of the abdominal wall that allows fat or abdominal contents to protrude through the abnormal opening. Common sites for hernias are the groin area, umbilicus, and prior incisions from abdominal operations. Some hernias that are not causing symptoms can be safely observed, but if a hernia is painful or associated with symptoms suggesting bowel obstruction, repair is mandatory. Laparoscopic techniques are often used to repair hernias and in many cases lead to a faster recovery. Laparoscopic repairs should only be performed by experienced laparoscopic surgeons. Not all hernias are suitable for a laparoscopic repair. For hernias that are not suited for a laparoscopic approach alternative modern tension free mesh repairs have excellent results. MGH general surgeons are skilled in assessing hernias and determining their appropriateness for open or laparoscopic surgery.

While the repair of most hernias is straightforward, recurrent hernias and large incisional hernias can require special techniques. In many cases, lifestyle modification is an important part of treating hernias. Smoking cessation and weight loss should be undertaken prior to repairing incisional hernias and umbilical hernias. In multiple recurrent hernias, tissue transfer may be required to rebuild the abdominal wall. In particularly difficult situations, a multidisciplinary team of plastic surgeons and general surgeons may be required for complex reconstructions. Surgeons from MGH Division of General Surgery are experienced with these types of complicated cases and routinely help patients determine and coordinate the most appropriate course of care.
