Make an Appointment

Thank you for choosing Massachusetts General Hospital. In order to schedule an appointment, please call the physician's office directly, and the office staff will be more than happy to assist you. Please remember that you may be required by your insurance company to obtain a referral from your primary care physician ahead of time.

When you come to your appointment, please bring your insurance card and your hospital blue card with you. If you do not have a blue MGH card, or have lost yours, please obtain one before the time of your office visit by registering with the hospital on the ground floor of the Wang Ambulatory Care Center. To assist the doctor in assessing your problem, you may receive questionnaire forms in the mail to fill out about your health condition, or you may be asked to fill these out when you arrive in the office. It is especially important to indicate clearly all the medications you take, including the dose in milligrams (which is indicated on the bottle). It is also important to include the full name and address of your primary care physician, plus any other physicians that you regularly see, so that a report may later be sent back to them. Also, if you have received treatment or tests for your condition at another institution, it is very helpful if you obtain the reports for your surgeon to see.
