Providing Continuity of Care
- A Nurse/Social Worker team supervises the health care program of each
client and teaches the host family about the client's needs
- Family Care Program staff make regular home visits
- Staff maintain close contact with each host family to offer assistance
and support
Establishing Strong Relationships with Families
- Our team works closely with host families to meet the needs of each client
- We make certain that each family feels appreciated and valued in offering
their home to the Family Care Program

- Frailty due to aging
- Chronic medical conditions
- Memory impairment
- Some types of mental illness
