Diabetes Self Management Education classes are offered for Type 2 adult patients.
The DSME program at the Revere Health Center consists of an individual visit with a nurse practitioner or nurse educator. From here, participants may proceed on to a series of group education sessions facilitated by a diabetes educator. Patients are encouraged to bring a family member or friend for support.
All group classes meet in the Sweet Conference Room on the first floor of the health center.
