The Diabetes Self Management Education (DSME) program at the Charlestown Community Health Center has been Recognized by the American Diabetes Association for Quality Self-Management Education*.
The DSME program at the Charlestown Health Center begins with a comprehensive individual visit with a diabetes nurse educator. Participants may then proceed to a series of group education sessions or one-on-one visits facilitated by a diabetes educator. Patients are encouraged to bring a family member or friend for support.
Diabetes Self Management Education classes are offered for Type 1 and 2 adult patients in English.
All classes meet in the Education Center in the basement of the health center.
