The DSME program at the Chelsea Health Center begins with a comprehensive individual visit with a diabetes nurse educator. Participants may then proceed to a series of group education sessions or one-on-one visits facilitated by a diabetes educator. Patients are encouraged to bring a family member or friend for support.
Diabetes Self Management Education classes are offered for Type 1 and 2 adult patients in Spanish or English. We are also offering a group class offered in American Sign Language.
