The DSME program at the Bulfinch Medical Group consists of an individual visit with a nurse practitioner or diabetes educator. From here, participants may proceed on to a series of group education sessions facilitated by a diabetes educator. Each program participant will also receive a 1:1 visit with a Registered Dietician. Patients are encouraged to bring a family member or friend for support.
Diabetes Self Management Education classes are offered for Type 2 adult patients. Group education sessions are scheduled one at a time. All groups will meet in the Educational Learning Center at Bulfinch Medical Associates.
