MassGeneral Care-Management Program is a three-year program for selected seniors who receive care at the Massachusetts General Hospital. The goal of MassGeneral Care-Management Program is to help patients with chronic and serious health conditions to stay healthier longer. If you decide to participate in the program, a customized healthcare plan will be developed just for you. You can take advantage of as many or as few services in the program as you’d like. It might include such things as home visits, phone calls to check up on you and help with any issues that come up related to your health. As always you’ll have access to the latest innovations and treatments for your health condition. And you’ll have direct access to a nurse/case manager, and more!

Massachusetts General Hospital and its physicians developed the MassGeneral Care-Management Program to help take the lead on developing innovative programs that could serve as a model of care for other seniors. Massachusetts General Hospital was one of few select centers in the nation invited to develop such an initiative.

If you’ve been invited to participate in the MassGeneral Care-Management Program, congratulations! This program is "by invitation only". You’ve been selected among many patients who receive care at Massachusetts General Hospital. You decide whether you’d like to participate—it isn’t mandatory and doesn’t change your relationship to your primary care doctor or your insurance benefits.

Your care will continue to be managed through your primary care physician’s office. The relationship you have with your primary care physician and other healthcare specialists at Massachusetts General Hospital stays intact. MassGeneral Care-Management Program doesn’t replace your relationship with your primary care physician or other healthcare specialists. It’s really an extension of their offices.

There is absolutely no cost whatsoever beyond your regular Medicare premiums, copayments, and deductibles at any time for seniors who elect to participate in the program.

MassGeneral Care-Management Program is NOT an insurance program. If you choose to participate in MassGeneral Care-Management Program, you will not be assigned to an HMO, Medicare Advantage plan, or other non-fee-for-service health plan. You remain in your original Medicare plan.

Also, you will NOT lose your Medicare, Medicare Part D, or supplemental insurance by joining the MassGeneral Care-Management Program. The MassGeneral Care-Management Program does not replace any of your current benefits. Your Medicare, pharmacy and supplemental insurance benefits, premiums and co-pays remain intact. You do not need to let Medicare know if you participate in the program.
