The Bulfinch Medical Group Diabetes Program began in January 2004 and was developed in recognition of the need patients have to be equipped, supported and educated in how to manage their diabetes. The program is based on the recognized National Standards for Diabetes Self-Management Education and the Diabetes Standards of Medical Care. It has been awarded American Diabetes Association Education Program Recognition and a Partners in Excellence Award. Diabetes management, coaching and education are offered in both individual and group settings by a team of Advanced Practice Nurse Practitioners, Registered Nurses who are certified in diabetes self-management education and a Registered Dietitian. This team works closely with the BMG primary care doctors to offer the BMG patient with diabetes and their families comprehensive diabetes care.
