How to Make a Referral

To refer a new patient, please fill out the following New Patient Referral Form and fax it it to 617-726-3441. If the patient is receiving chronic opioid therapy, please include the Consultation Services for Patients Receiving Chronic Opioid Therapy form on page 2.
